Fever, Body Aches or Chills? The Powerful Benefits of Same-Day Rapid Tests for Dengue, Flu & COVID-19

A rapid fever test can help your doctor identify possible causes of fever, body aches and chills more quickly, especially when illnesses such as dengue, influenza (flu) and COVID-19 can cause similar symptoms.

In Malaysia, these infections are important considerations when someone develops an acute fever. Because their early symptoms can overlap, symptoms alone may not always tell you exactly what is causing you to feel unwell.

Getting assessed early allows your doctor to review your symptoms, examine you and decide whether tests for dengue, influenza or COVID-19 are appropriate or not.

Why Fever, Body Aches and Chills Can Be Difficult to Tell Apart?

Fever is not a disease by itself; it is a symptom that occurs when your body is responding to an infection or another medical condition.

Dengue, influenza and COVID-19 may share similar symptoms such as:

  • Fever
  • Chills
  • Headache
  • Muscle or body aches
  • Tiredness
  • Reduced appetite
  • General feeling of being unwell

There can also be clues that make one infection more likely than another.

Dengue may present with fever together with headache, muscle aches, joint pains, pain behind the eyes, rash or bleeding manifestations.

Influenza commonly causes a relatively sudden onset of fever, chills, muscle aches, headache, fatigue, cough and sore throat.

COVID-19 can also cause fever, cough, sore throat, fatigue and body aches, among other symptoms.

However, there is enough overlap that it is not advisable to diagnose yourself based on symptoms alone.


Rapid Fever Test: Why Early Testing Can Be Helpful

A rapid fever test is not necessarily one single test. Depending on your symptoms, duration of illness, examination findings and exposure history, your doctor may recommend testing for one or more infections.

For example, this may include:

Dengue rapid testing
Dengue testing may look for NS1 antigen and/or dengue antibodies. Importantly, the usefulness of these markers changes according to the day of illness.

Influenza rapid testing
A nasal or nasopharyngeal swab may be used to look for influenza A and B.

COVID-19 testing
An antigen rapid test (RTK-Ag) may be used in appropriate circumstances to detect SARS-CoV-2 infection.

Rapid tests can provide useful information during the consultation, but the result should always be interpreted together with your symptoms, examination findings and the timing of your illness.


Dengue Rapid Test: Timing Matters

This is particularly important for dengue.

According to the Ministry of Health Malaysia’s Clinical Practice Guidelines for dengue, different dengue tests become useful at different stages of the illness.

NS1 antigen is particularly useful during the early stage of dengue infection. Dengue antibody tests such as IgM become more useful later in the illness.

This means that a negative dengue test performed very early does not always completely rule out dengue.

Depending on your symptoms and the day of illness, your doctor may recommend:

  • Repeat dengue testing
  • Full blood count (FBC)
  • Haematocrit monitoring
  • Platelet monitoring
  • Further clinical review
  • Referral to hospital when clinically indicated

This is why knowing which day your fever started is extremely important when you see your doctor.


Why a Negative Rapid Test Does Not Always Mean “No Infection”

Rapid tests are helpful diagnostic tools, but they are not perfect.

A negative result can occur when:

  • Testing is performed too early or at a less suitable stage of illness
  • The amount of virus or antigen present is below the test’s detection threshold
  • The illness is caused by another infection
  • The clinical condition is evolving

For dengue in particular, KKM guidance emphasises that laboratory results should be interpreted according to both the patient’s clinical presentation and the timing of testing.

Therefore, if your rapid test is negative but your fever persists or your condition worsens, you may still need another medical assessment.


Positive Test? Faster Identification Can Guide the Next Step

One of the biggest advantages of same-day testing is that it can help your doctor make a more informed management plan.

If Dengue Is Suspected or Detected

There is no specific routine antiviral treatment that cures dengue. Management is mainly supportive and depends on the patient’s clinical condition.

Monitoring is particularly important because dengue is a dynamic illness that progresses through febrile, critical and recovery phases.

Your doctor may advise hydration, appropriate fever treatment, blood monitoring and follow-up. Patients with warning signs or signs of severe dengue require urgent assessment and may need hospital management.

If Influenza Is Detected

Treatment depends on your symptoms, duration of illness and individual risk factors.

Your doctor may recommend supportive treatment and, when clinically appropriate, antiviral medication. Identifying influenza can also help avoid unnecessary antibiotic use because antibiotics do not treat influenza viruses.

If COVID-19 Is Detected

Management depends on the severity of illness and the patient’s individual risk factors.

Most mild infections can be managed symptomatically, while patients who are at higher risk of complications or who develop concerning symptoms may require further assessment or hospital care.


Do I Need Antibiotics for Fever?

Not necessarily.

Dengue, influenza and COVID-19 are caused by viruses. Antibiotics do not treat these viral infections.

Antibiotics should only be prescribed when your doctor suspects or identifies an appropriate bacterial infection or to prevent secondary bacterial infection. 

Testing and clinical examination can therefore help your doctor determine a more appropriate treatment plan rather than prescribing medication based on fever alone.


When Should You Consider a Same-Day Fever Assessment?

Consider seeing a doctor if you develop symptoms such as fever with:

  • Significant body aches
  • Chills
  • Headache
  • Cough or sore throat
  • Unusual tiredness
  • Rash
  • Pain behind the eyes
  • Persistent vomiting
  • Reduced oral intake
  • Symptoms that are worsening rather than improving

It is particularly important to seek medical advice if you have recently been exposed to someone with influenza or COVID-19, or if dengue is circulating in your residential or workplace area.


Dengue Warning Signs: When Fever Becomes More Concerning

An important point about dengue is that feeling less feverish does not necessarily mean the danger has passed.

Clinical deterioration can occur around the critical phase of dengue illness.

Seek urgent medical attention if you develop concerning symptoms such as:

  • Persistent vomiting
  • Severe or increasing abdominal pain
  • Bleeding
  • Marked lethargy, restlessness or weakness
  • Difficulty breathing
  • Dizziness, fainting or signs of poor circulation
  • Inability to drink adequately
  • Reduced urine output
  • Rapid deterioration in your general condition

Patients with severe symptoms should not wait for a clinic rapid test before seeking urgent medical care.

What Happens During the Consultation?

1. Medical history
